Audiology CEU Submission

Please Note: You will only be able to submit for Audiology CEU credits one time. Please submit this form once you have attended all your sessions and acquired each session’s CEU code.

CEU forms must be submitted no later than Friday, November 17, 2023.

CEU credits will be finalized and a certificate will be delivered around December 20, 2023.
